Fee Schedule
Services provided are out of network with insurance. You can submit your bill to your insurance company. Insurance companies will then reimburse based on your out-of-network benefits. Please contact your insurance company to review your out-of-network benefit details.
For new patients, $120 per consultation and $60 for each follow up visit. Problem based visit fee for established patient is $60.
Subscription is $90/month or $850/year.
